I am running latest version 7.2.4 too.
Here is the file name I have listed in Image output (STATEMENTS.STATEMENTS-PDF_%m%d)

That's strange... that's also what I've used to test the file name you gave us, and that worked for me.
One thing I just thought about though... do you have file extensions displayed in Windows? By default, they are hidden when you install Windows. Just as a test, if you go in the Windows Control Panels, and then in the Folder Options, under Display, try unchecking the options to hide the file names extensions. This means that the file name is correct, just not displayed properly(which would be due to a confusion of using a dot after STATEMENTS...)
Another thing to try: if you use another character than a dot(like a dash, for example), does it works?

Raphael, I checked the Windows control panel and the hide file extensions was already unchecked. However, I went ahead and tried checking it and running test (which did not work). Then I unchecked it againa and reran test which also did not work again. Yes, I'm using a dash in the name and that is showing. Here is the latest of what I could create: STATEMENTS.STATEMENTS-PDF.pdf My date is being left off. I have it as .%m%d (STATEMENTS.STATEMENTS-PDF.%m%d)

Raphael, I changed it to Optimized PostScript and I had the same results. ????
